Thank you for deciding to enroll at the Academy!

Please click the button below (or in the sidebar) to complete the Academy’s online enrolment process.

Once the online enrollment forms are complete, there are several documents required to complete the enrollment. Please be aware that all of the documents must be submitted before enrollment is official.

1. Birth certificate
2. Immunization Records, including Statement of Varicella (Chicken Pox), or a signed waiver from your local Health Department
3. Most recent transcript OR 8th grade report card
4. Discipline records from most recent school
   1. We can start with dated print-outs from online portals like PowerSchool or Infinite Campus, but we will require a signed affirmation of prior discipline form from the administration of the previous school to finalize enrollment.
   2. We can fax a request for the discipline record.
5. If you child has received additional support services at a previous school, we will need a copy of the most recent 504 or IEP. Please provide a copy of the entire document.

We can accept original copies, emailed color scans of originals, or faxes from schools as official documents. We will copy and immediately return any original copies.

Other Documents

School staff may ask for the following when applicable.

1) Individualized Education Program (IEP)/Evaluation Report (ER)

2) A copy of the court order or custody agreement is required to be provided if the parents are separated or divorced and the enrolling parent is relying on the order or agreement as the basis for enrolling the child.
